# Experimental CAR-T Cell Therapy Cures 3-Year-Old Boy’s Liver Cancer

A 3-year-old boy with a chemotherapy-resistant liver cancer that had spread to his lungs and bones is in full remission after two doses of an experimental CAR-T cell therapy, according to a case report published September 15 in [The New England Journal of Medicine](https://www.nejm.org/doi/full/10.1056/NEJMc2605958). The treatment is the first documented complete regression of a solid tumor by CAR-T in a child. First of its kind.

CAR-T cell therapy works by drawing a patient's own immune cells, engineering them in a lab to recognize a tumor marker, and infusing them back to hunt cancer. Until now, the technique has mostly worked against blood cancers like leukemia and lymphoma, not solid tumors. When the boy was first diagnosed, the primary tumor in the left lobe of his liver measured 11.2 centimeters by 9.6 centimeters by 7.1 centimeters. Scans showed the cancer had metastasized to his lungs, with signs of bone involvement too. He went through three rounds of chemotherapy, had the primary tumor surgically removed, and had two operations to cut out the lung lesions. The tumor had been lethal. The cancer came back fast, with a new lung tumor appearing within weeks.

After the rapid recurrence, the boy was enrolled in the CARE trial, the first human study of an experimental CAR-T product called GPC3-CAR. The therapy targets glypican-3, a protein that is heavily expressed in liver cancers. Researchers also engineered the cells to express two immune proteins, interleukin-15 and interleukin-21, to extend the cells' lifespan and tumor-killing power, based on earlier preclinical work.

After the first infusion, scans showed a partial response, with alpha-fetoprotein levels in his blood falling. That protein tracks tumor activity. Eight weeks later he got a second dose. Follow-up scans then showed no remaining signs of disease apart from some residual scarring. Twelve months on, the boy is still disease-free. Pediatric oncologist David Steffin from Baylor College of Medicine, the paper's first author, called the result a durable complete response achieved entirely in the outpatient setting without systemic toxicity. The boy is doing well.

Researchers caution that this is a single case in an early-phase trial, and broader results will determine how effective the therapy is across other patients. The CARE study continues at Baylor, and a parallel trial called IMPACT is running at Seattle Children's Hospital and the University of Washington.
